Available Position: Speech Language pathologist 

Work Hours: This service is needed Part‐time (approximately 20 hours per week) in Northern Hamilton County and Part‐time (approximately 20 hours per week) in Southern Hamilton County.  These sites have a developmentally disabled population including some individuals who are medically or physically disabled as well as some who are confined to wheelchairs. 

The Speech Language Pathologist would be required to provide services at the following locations:
1. Long Lake Day Hab ‐ 16 Stone Road, Long Lake, NY 12847
2. Speculator Day Hab ‐ 2880 St. Route 30, Speculator, NY 12164
The distance between the facilities is approximately one hour.

Scope of Work:
A. Complete necessary evaluations and annual reports as indicated by the Individual Service Plan (ISP) with respect to needs of the individuals in the areas of communication disorders, therapeutic dining and swallow therapy.  The Speech Language Pathologist will develop individual specific programs in these specified areas and provide training and on‐going supervision to staff.  
B. Consult with team members regarding needs in other areas of communication disorders, therapeutic dining and swallow therapy.
C. Review and sign reports related to the ISP and provision of services to individuals.
D. On request of the Treatment Team Leader (TTL), Program Manager, Medicaid Service Coordinator and/or RN, provide in‐service education to the professional and direct care staff of the facility which would enhance the effectiveness of the Speech Language services including communication disorders, therapeutic dining and swallow therapy.
E. Provide therapy programs related to communication disorders, therapeutic dining and swallow therapy as specified in each individual’s ISP.
F. The Speech Language Pathologist is responsible for obtaining prior Medicaid approval and for ordering and maintaining any and all equipment related to the provision of services in communication disorders, therapeutic dining and swallow therapy.
G. The Treatment Team Leader, Active Treatment Coordinator and/or other designee agree to make all relevant information available to the Speech Language Pathologist.   
H. The Registered Nurse(s) or their designee will provide supervisory oversight, in‐servicing of the individual(s) for the requested procedure(s), and assure that all other necessary and relevant information and supplies are available to the Speech Language Pathologist providing appropriate services.
I. It is the responsibility of the Speech Language Pathologist to familiarize and complete all provided Sunmount DDSOO Policies and Procedures, as well as other educational materials provided by the Supervisor, inclusive of, but not limited to: 1. Individual rights 2. Infection control 3. Reporting requirements for suspected abuse/maltreatment
J. The Speech Language Pathologist’s performance will be monitored on an ongoing, as well as annual, basis by the RN(s), Team Leaders or other Sunmount DDSOO staff.   
K. There will be a book at both locations that the Speech Language Pathologist is required to sign in and out of on days worked.  A copy of the sign in and out log is required to be submitted with invoices.
