We are seeking a Dentist to provide services at the Dzilth-Na-o-Dith-Hle Health Center located in Bloomfield, NM. 

Work Schedule: 8:00 am to 4:30 pm, Monday, Tuesday, Thursday and Friday, and from 8:00 am to noon on Wednesdays.
Contract period: The required services are to commence upon credentialing and fingerprint clearance tentatively scheduled from January 1, 2019 to December 31, 2019. NOTE: Period of Performance may be available beyond 12/31/2019 if service is still needed, however, it will be re-competed at that time. 

-Provide quality dental services to patients of all ages 
-Diagnose and treat common dental disease, dental health issues, and emergency dental treatment. 
-Prepare treatment plans, provide recommendations on patients evaluated 
-Interpret digital radiographs 
-Evaluate, review clinical records and treat patients with great customer service. 
-Adhere to records documentation procedures 
-Deliver care in accordance with accepted standards of care and hospital policy procedures. 
-May be subject to technical direction and/ or guidance of the designated Government Official when appropriate. 
-Provide other duties assigned that are consistent with privileges of the Medical/Dental Staff Governing Body of the Northern Navajo Medical Center. 

Client background: 
Many of the patients service may only speak a native language and reside on the reservation. The contractor should be able to show sensitivity to cross cultural and language differences. The contractor must have the ability work with interpreters in some cases for non-English speaking patients. In situations, where interpretation is required, the Northern Navajo Medical Center staff will provide these services. 

Clinical Privileges: 
The contractor must be credentialed and clinical privileges granted by the Northern Navajo Medical Centers Chief Medical Officer or designee prior to providing services. The contractor must obtain the appropriate forms for privileging from the Northern Navajo Medical Centers Chief Medical Officer upon award of this contract. 
Contractor shall furnish current immunization or immunity for MMR (Measles, Mumps, and Rubella), Td/Tdap and Hepatitis B status, or as required by the Navajo Area Indian Health Service immunization policy. A copy of the document shall be submitted to the Employee Health Nurse upon arrival at the facility before providing service. 

1. The Provider shall possess a valid, current, and fully unrestricted license form any state, District of Columbia, the Commonwealth of Puerto Rico, or Territory of the United States.
2. Must possess a current and fully unrestricted DEA license.
3. Two (2) Letters of Recommendation with CURRENT contact information.
4. Proof of Basic Life Support Certification.
5. Proof of current immunization.
6. Health status allowing physical and mental ability to carry out required functions. This is to be verified on an annual basis by the acquiring agency or a copy of a physical examination must be provided.
