We are seeking an experienced Pharmacist for a contract position at the Sagamore Children's Psychiatric Center in Dix Hills, New York.

Client Facility: Sagamore Children's Psych Center, 197 Half Hollow Rd, Dix Hills, NY 11794

Work Shift:  Mon through Friday 8am to 4:30pm. Pharmacy closed on weekends and holidays.
 
Assignment Duration: 10/15/2018 to 09/30/2018

Scope of Work: 
The pharmacist is responsible for providing quality pharmaceutical services to pediatric patients in the hospital. This includes the procurement, storage, preparation, accountability, and dispensing and disposal of medications and pharmaceuticals according to pharmacy standards, regulations, policies and procedures. The pharmacist will inspect medication area’s where medications are stored in the facility. He or she should be familiar with the clinical diagnosis and medical management of the patient. The judicious use of medications is a function the pharmacist shares with the Practitioner, nurse, and other members of the health care team.
The pharmacist is responsible for keeping an up to date patient medication profile and monitor the patient's medication therapy to detect possible drug interactions, adverse drug reactions and incompatibilities, allergic reactions, and to monitor safe therapeutic doses.
The pharmacist should be familiar with the pharmaceutical resources and literature available so that he or she can make decisions concerning drug information and the patient's medication therapy as well as provide patient education.
The pharmacist is expected to use sound professional judgment at all times and to be able to conduct inter-professional relationships and communications. He or she must perform as a member of the patient's health care team and make contributions to the patient's treatment.
He or she is also responsible for maintaining his or her professional knowledge thru continuing pharmacy education, including the latest information concerning Pharmacy laws and regulations.
The pharmacist must at all times practice and dispense medications in accordance with all federal, state, and local agencies, and facility pharmacy policies and procedures. He or she is responsible for maintaining all required Pharmacy records as directed by rules and regulations.
Number of Scripts per day: variable. Usually from 50-200
Team layout: A Pharmacy Director, a Staff pharmacist and Pharmacy Technician 
Children age group: Ages 9-17

Facility Information: 
Sagamore's programs for youngsters and their families include inpatient hospitalization, day hospitalization, day treatment, outpatient clinic treatment, outpatient clinical support services, mobile mental health teams, crisis services, information and referral, and community training. The inpatient facility is based in Dix Hills as is the Day Hospital. Sagamore operates the Waverly Clinic in Patchogue  as well as four Day Treatment programs. Three of these are located in Suffolk County (Bellport, North Babylon, and Sayville) and one in Nassau County (Wantagh). Mobile Crisis Teams operate in Suffolk County and respond to psychiatric emergencies in homes and in schools. A liaison is present in the Family Court to assist with mental health screening.
Area Served: Nassau and Suffolk Counties on Long Island.
Age Groups Served: Children served are between the ages of 6 and 17. Exact age varies by program.

Qualifications:
The pharmacist must have a Bachelor's, Masters or Pharm D Degree in Pharmacy from an accredited college of Pharmacy or graduation from a Pharmacy program;
Must posses a New York State license and current registration.
Pharmacist with previous Psych experience is pre What Software do you use?
Experience with ES Vista is desired.
