Facility Information:  The Fort Yuma Health Care Center is an outpatient ambulatory care center accredited by the Accreditation Association for Ambulatory Health Care (AAAHC). Fort Yuma Health Care Center outpatient services include general medical, pediatric, dental, hospital discharge follow-up, and a variety of specialty clinics. Outpatient visits for the Fort Yuma Health Care Center totaled 44,360 for Fiscal Year 2017. The Fort Yuma Health Care Center work site is primarily an outpatient ambulatory care center providing services to about 44,000 patients per year. The services include general medical, pediatric, community health, preventative health, and mental health. Dental care is also available. Special ambulatory services are provided on-site by IHS staff based in Phoenix or professional medical locum tenen contractors. FYSU uses a multi-disciplinary team approach to patient care; the Service Unit also provides community-public health programs/services, including public health nursing, social services, health education, and environmental health. The Fort Yuma Health Care Center is accredited by The Accreditation Association for Ambulatory Health Care (AAAHC).

Work Schedule: Pharmacist shall be scheduled to work 8 am-5 pm Monday-Friday. Fort Yuma Health Care Center is closed on weekends and holidays.

Contract Period of Performance: The Contractor is requested to provide Fort Yuma Health Care Center, for 10 weeks

Scope of Work: The Contractor shall provide medically necessary routine Pharmacist Services for the Fort Yuma Service Unit

Qualifications:
Education: Either a Bachelor of Science Pharmacy degree, or Doctor of Pharmacy degree recognized by the Accreditation Counsel for Pharmacy Education (ACPE).
Experience: The contractor shall provide the Government with a copy of their resume/curriculum vitae for their file. The Contractor file will be kept by the Department of Pharmacy for verification of licensure and/or certificates. The file will also contain personal information such as home address, phone number, next of kin, emergency contact, health requirements, mandatory in-service and continuing education, competency assessments, and awards or disciplinary actions.
Licensure: The Contractor will hold a current, valid and unrestricted pharmacist license in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. The Contractor shall present an original, current license when reporting for duty to the Pharmacy Department, or reporting for orientation, whichever comes first. A copy will be placed in the Contractor file. No reimbursement will be made for a Pharmacist who reports to work with an expired license, or one that is not in good standing with that State’s Board of Pharmacy.
Certification: Current certification in BLS (Basic Life Support) is required.
