Work Hours: 8 hours per day; Shifts: 8:00am-4:30pm Monday - Friday with NYS Holidays Off

Scope of Work: 
The primary function is to provide oversight required to manage the cases assigned to ensure appropriate medical care is received.
The RN will be required to attend a two week training program.
Then the RN will be required to report to the assigned field office to review calls/messages from the after hours on call service.
The RN will be required to visit the individuals at their home or day program a minimum of weekly to triage priority needs based on individual health status, and staff training. Priorities include but are not limited to assessing emergencies, review the need to start/stop/adjust medications, need for appointments, ensure completion of appointments and participate on specialty appointments as needed, make hospital visits for discharge planning and attend team meetings.
The RN will be expected to review pending consult packets for completeness, ensuring labs, radiology or other diagnostic data are attached as needed. Completed packets will be reviewed to ensure the consult has been completed and all attachments are present (e.g. office visit notes, prescriptions for referrals etc) and follow up appointments are made. Ensures copies are sent to the referring provider and appropriate team members (e.g. dietitian, therapists etc). Consults are to be reviewed for completeness, signed and filed appropriately.
The RN will review documentation such as progress notes, communication books and various clinical logs (bowel regime, seizure, menses, vital signs etc), Medication and Treatment administration records, and sign all documents reviewed. The RN will document: all assessments, follow-up care, communication with staff, providers, service coordinators and family members, hospital reports will be filed, Plans of Nursing Service written and any health related training to unlicensed staff/caregivers and follow-up for any deviations in expected practice such as medication events.

Assignment Duration: 03/15/2018 until 3/14/2020
A current State of New York RN license is required.
Previous experience working in the community and/or with persons with disabilities is preferred.
