We are seeking a 
Optometrist .

Tour of Duty: 
Tour of Duty: 8:00 am to 5:00 pm, Monday through Friday, Forty (40) hours per week, Call-Back Hours and Overtime Hours after 5 pm. Overtime: Contractor shall be subject to working longer hours if patient load demands are present and rotating shifts to provide coverage in the Optometry Department. The contractor shall work 100% of the contracted hours. Works shifts as arranged by Chief of Optometry. The contractor shall rotate into other duty sections as needed to support patient care.

Scope of Work:  
Provide professional medical services or direct patient care services:
The contractor shall:
A. Diagnose vision impairment and ocular disease through examination of the eye and adnexa.
B. Treat and manage vision impairment and ocular disease with corrective lenses, therapeutic pharmaceuticals and other appropriate means.
C. Use all forms of topical ophthalmic therapeutic medicine and oral therapeutic medicine to manage pathology and trauma.
D. Provide emergency eye care services including the diagnosis and treatment of ophthalmic trauma and pathology consistent with hospital privileges.
E. Utilize laboratory and radiology services to evaluate ophthalmic and related systemic conditions. F. Direct optometric technicians to assist in the clinical examination as appropriate.
G. Provide appropriate documentation on rendered patient care utilizing CCHCF’s electronic health record (EHR) system. The provider is responsible for utilizing the CCHCF EHR system to obtain, dictate, electronically enters and assures preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Clinical data reports shall be completed in a timely manner and shall be maintained in compliance with Joint Commission (JC) standards. It is the responsibility of the provider to use the training resources provided by the CCHCF to develop competency in utilizing the EHR system.
H. Coordinate systemic care of ocular disease patients with other physicians in the hospital.
I. Examine inpatients as requested by attending physicians.
J. Consult with Unit Chief of Optometry and/or private ophthalmologic consultants and medical practitioners as needed for complex cases.
K. Refer and coordinate contract health care for patients requiring medical, surgical and/or diagnostic services outside of the CSU and assists in the coordination of patient transport for urgent/emergent ocular conditions.
L. Provide consultation on difficult cases to practitioners in the same or other specialties. Provides teaching to medical practitioners, residents and students on the most complex patients.
M. The contractor shall perform additional Computer Applications & Administrative Duties.

Position Requirements/Qualifications
Training/Experience. The Contractor shall have completed an accredited Optometry Residency Program with a minimum of six months experience in the specialty required by the contract, unless otherwise approved by the CCHCF’s Credentials Committee. There must be no lapse of services of no more than six (6) months prior to beginning tour of duty with CCHCF.
Certification/License/Registration. The Contractor shall be board certified with the National Board of Examiners in Optometry, and shall possess a current, valid, unrestricted Optometry license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract. If the contractor is asked to perform outside the normal training parameters of his/her specialty, then the contractor shall provide certification of hospital privileges in those additional scope of practice(s).
Drug Enforcement Administration (DEA) License. The contractor shall maintain a valid, current, full and unrestricted DEA license with all schedule types (II through V) and maintain certification throughout the term of the contract. 6.2.2 The contractor shall maintain a valid and current Cardiopulmonary Resuscitation (CPR) and Basic Life Support (BLS) certifications and are mandatory.
