We are seeking a Certified Nurse Midwife at Chinle Hospital  from 12/01/2017 through 10/31/2018.

Tour of Duty: Works shifts according to the monthly Certified Nurse Midwifery Schedule. Tour of Duty: 8 a.m. to 5 p.m., Monday through Friday, Forty (40) Hours per week OR Tour of Duty: 7:00 a.m. to 7:00 p.m. or 7:00 p.m. to 7:00 a.m. Sunday through Saturday including holidays, 12 hours per shift. Overtime: Contractor shall be subject to working longer hours if patient load demands are present and rotating shifts to provide coverage in the Obstetric Care Unit (OCU).

Scope of Work:  
The contractor shall manage the medical and midwifery care of patients in the antepartum period, and during labor and delivery. Management defined as: “evaluating the patient to determine needs and/or problems, planning a course of action to meet the needs, providing direct care to patients, and/or insuring that the plan of action is carried out through others”.
The contractor shall manage the medical/midwifery care of patients during their postpartum period (clinic and inpatient), provide family planning education/services, and gynecological service to healthy women.
The contractor shall admit patients, obtain and record medical histories, perform physical examinations, determine plan(s) of care and write orders to carry out the plan(s)
The contractor shall order, administer and monitor medications within prescribed guidelines. Under the guidelines, orders for medications are honored by nursing and pharmacy staff without counter-signature of a physician.
The contractor shall provide well childcare as determined by CCHCF’s policy.
The contractor shall evaluate and determine the need for consultation and obtains physician consultations as needed. Shall implement consultant management plan.
The contractor shall collaborate with physicians on planning and implementing care for selected patients at high risk.
The contractor shall determine the need to involve health care providers, obtain consultations, and work cooperatively with them.

Position Requirements/Qualifications
Experience/Training. The contractor shall have completed an accredited certified nurse midwifery program with a minimum of six months experience in the specialty required by the contract, unless otherwise approved by the CCHCF’s Credentials Committee. There must be no lapse of services of no more than six (6) months prior to beginning tour of duty with CCHCF.
Certifications/License/Registration. The contractor shall be board certified with the American College of Nurse Midwifery, and shall possess a current, valid, unrestricted appropriate license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract. If the contractor is asked to perform outside the normal training parameters of his/her specialty, then the contractor shall provide certification of hospital privileges in those additional scope of practice(s).
Drug Enforcement Administration (DEA) License. The contractor shall maintain a valid, current, full and unrestricted DEA license with schedule types II through V and maintain certification throughout the term of the contract.
The contractor shall maintain a valid and current Cardiopulmonary Resuscitation (CPR), Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) and Advanced Trauma Life Support (ATLS) certifications and are mandatory for midwifery.

Health Requirements/Conditions of Employment:​
Medical Evaluation.
The contractor shall provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
Immunization. The contractor shall provide documentation as evidence of immunization as required by NAIHS immunization policy for the following:
Immunity to Rubella, Mumps, Measles
Immunity to Hepatitis B series
History of chicken pox (varicella) disease or positive titer
Tetanus Diphtheria (Td) and Pertussis within the last 5 years
Influenza within the last year
Documentation of receiving a TB Mantoux skin test (PPD) within the past 12 months with documentation of follow-up for a positive test.
A signed declination of the Hepatitis B vaccination series will be accepted.
